Important Notice - Instructional Level II Application Requirements

   This update applies to all school districts employing individuals who received their Level I certificate on or after September 1, 2001, and will be ready to make application for a Level II certificate for these employees, on or after September 1, 2004. All such Level II applications must result from, and contain reference to, the satisfactory results reported through the use of the Pennsylvania Department of Education (PDE) 427 (Instructional I to Instructional II Assessment Form).

   The state form used in the actual Level II application, PDE 338P, Application for Level II certificate, has been updated as of January 2004, and now includes an initialed check-off to verify the use of the PDE 427 form and satisfactory rating. However, if using a copy of the PDE 338P that was printed prior to January 2004, there is no initialed check-off included. In this case, the district must include the last page of the PDE 427 with the application for a Level II package. The updated PDE 338P application form is available on our web site at www.pde.state.pa.us.

   Any applications submitted without addressing the required PDE 427 assessment information will result in a returned letter to the applicant requesting this missing component as of September 1, 2004.
